Support Coordination

With our support coordination service, you will understand and implement the funded supports in your plan and better link to your community, mainstream, and other government services. A Support Coordinator will focus on supporting you to build skills and direct your life as well as connect you to providers.

Your Support Coordinator will assist you to negotiate with providers about what they will offer you and how much it will cost out of your plan. Support coordinators will ensure service agreements and service bookings are completed. They will help build your ability to exercise choice and control, to coordinate supports and access your local community.

Specialist support coordinators will support you to manage challenges in your support environment which may include health, education, or justice services. Specialist support coordination aims to reduce barriers to implementing or using your NDIS plan.

Who is NDIS Support Coordination for?

NDIS Support Coordination provides support to people with NDIS packages seeking to understand which services they require to take positive steps in their mental health recovery journey. We can help you get the best out of your NDIS plan. Our NDIS Support Coordinators can help you to: 
-understand your goals and find the services you need to achieve them  
-develop service agreements with providers 
-self-advocate for the supports you need and navigate NDIS plan reviews.

Support Coordination: Level 2

A support coordinator will work side by side with you to strengthen your ability to coordinate and implement supports in your NDIS plan so you can participate more fully in the community. This involves working together to understand your plan & funding, identify what you expect from services and connecting you to the best services for your needs. A support coordinator will help develop your capacity to implement and manage your NDIS supports more independently over time.

Support Coordination: Level 3

A Specialist Support Coordinator will work side by side with you to address any barriers impacting your ability to implement your NDIS plan and access appropriate supports. This involves working together to understand your plan & funding, identify what you expect from services and connecting you to the best services for your needs. A Specialist Support Coordinator will negotiate appropriate support solutions and assist you to access to relevant supports during crisis situations.
